Our lovely practitioners will be able to help with the following issues: Health & Wellbeing, Loneliness, Mental Health, Bereavement, Long-Term Health Conditions, Social Care Needs, Physical Health, Weight Issues, Financial Problems and can issue food bank vouchers.
They can see patients up to four-times and will be able to on-refer.
